❗ Why would a claim be denied?

Claims are typically denied for one of these reasons:
    •    your plan does not include nutrition counseling
    •    your deductible has not been met
    •    a referral or authorization was required but not provided
    •    you reached your visit limit
    •    your insurance information was incorrect or inactive


💳 What happens if my claim is denied?

If a claim is denied:
    •    the session may be billed as out-of-pocket
    •    you may receive a bill after processing
    •    your responsibility will depend on your plan


🔍 How to understand your denial

👉 The best place to check is your Explanation of Benefits (EOB)

This will show:
    •    why the claim was denied
    •    what your insurance covered (if anything)
    •    what you may owe


🧭 What should I do next?

If your claim was denied:
    1.    Review your EOB for the denial reason
    2.    Confirm your benefits with your insurance provider
    3.    Check that your insurance information is correct
