Before your first nutrition appointment, you may be asked to complete intake forms in your Client Portal. These forms help your dietitian understand your health history, nutrition habits, and goals so they can provide personalized support during your session.

Completing your forms ahead of time helps ensure your appointment runs smoothly and allows your dietitian to make the most of your time together.

Where to Find Your Intake Forms

You can access your intake forms through your Client Portal.

Once logged in:
    1.    Navigate to your Forms or Tasks section.
    2.    Select any forms marked as incomplete.
    3.    Complete all required fields.
    4.    Submit the form once finished.

If forms are required before your first appointment, they will appear in your portal after your account has been created.

What Information These Forms Include

Your intake forms may ask for information such as:
    •    Health history
    •    Current medical conditions
    •    Medications or supplements
    •    Food preferences and eating patterns
    •    Lifestyle factors such as sleep, stress, and activity
    •    Your goals for nutrition counseling

Providing accurate information helps your dietitian better understand your needs and tailor their recommendations to you.

When to Complete Your Forms

We recommend completing your intake forms at least 24 hours before your appointment whenever possible.

This allows your dietitian time to review your information ahead of your session.

If forms are incomplete, you may be asked to finish them before your appointment can begin.

If You Cannot Find Your Forms

If you are unable to locate your intake forms in your Client Portal:
    •    Try refreshing your browser or logging out and back in.
    •    Confirm that you are logged in with the email used to create your account.

If you still cannot locate your forms, please contact our support team for assistance.
